Service Facilitation

 

Ward Transitional Services provides consumer-directed personal and respite care service facilitation to individuals on the CCC Plus Waiver, DD Waiver Services, including Community Living and and Family & Individual Support Waivers managed by the Department of Behavioral Health and Development Services. 

Consumers receive assistance with activities of daily living such as bathing, eating, dressing, housekeeping, as well as meal preparation, shopping, and laundry. Assistance is also given in the community while an individual works

or attends medical and social appointments.

 

Consumer-directed services, often called self-directed services, empower individuals with disabilities and older adults to have control over the services they receive.

Consumer Direct Care Network Virginia

Consumer Direct Care Network Virginia serves as the Fiscal/Employer Agent (F/EA) for qualifying older adults and those with intellectual and developmental disabilities who self-direct their in-home care. We specialize in services and support that allow your choice and control to live successfully in your home and community.

Public Partnerships LLC

Public Partnerships | PPL supports you and your family in that choice to remain in your home and community. As Medicaid law in most states prohibits direct cash payments, our role as a Financial Management Service (FMS) provider, is to help you in your role of the employer of your own support workers.  We assist you in hiring, paying for your choice of support workers and services, all within your state-approved personalized spending budget and local rules.

ACES$ Financial Management Services

ACES$ provides Medicaid recipients receiving Home and Community Based Services (HCBS) and their caregivers with a Consumer-directed payroll management solution. If you are on a HCBS waiver we serve, you can get the ACES$ Advantage.

Waiver Services Rep

Waiver Services Representative

The role of the Waiver Services Representative is to monitor all in-home services provided to consumers receiving waiver services (EDCD, CL, FIS). The Waiver Services Representative ensures satisfaction of services, health and

well-being of the consumer served, and safety while the consumer remains independently in the community. Travel is required for this position.

Responsibilities and Duties

  • Complete monthly in-home visits to consumers receiving waiver services.

  • Documenting visits using appropriate DMAS documents and company’s CSM database.

  • Identify needs and make necessary referrals into the community.

  • Support the consumer in developing and monitoring Consumer-Directed Services in the home (personal care, respite) as based on the service plan.

  • Provide necessary training to the consumer and attendants as needed to ensure a positive employer/employee relationship.

  • Assist consumers with hiring the person of their choice to provide daily living needs with no cost to the consumer. All services are paid by Medicaid.

  • Learn and understand all waiver programs and the necessary DMAS documentation needed to enroll and maintain services.

Qualifications and Skills

  • Minimum of Associates degree in a health or human services field, per Medicaid guidelines.

  • Experience working with the elderly and disabled for 1-3 years.

  • Use appropriate communication with consumers, providers, and agencies as needed while returning all messages within 24 hours.

  • Must have good organization and time management skills.

Requirements of the position

  • Reliable transportation and a valid Driver’s License. Travel is required for this position.

  • Successfully pass two background checks with no barrier crimes.

  • Access to internet in home, as work from home is required.

  • Experience with Microsoft products.

  • Cell phone.

  • Laptop and/or device that can access internet.

  • Physically able to walk up and down ramps and steps.

  • Physically able to carry 10 pounds.
